Simply copy/paste the below form into your blank email, fill in your group sheet information, and send to:

The chart below is made for a family of 15 children. It is only 2 generations since there is no simple way to do more than
that and store it. To simplify filing these and prevent duplication, we will only be filing the chart under the *father's* surname.
We hope that you will also submit a separate chart on the mother's family (mom, dad & siblings). If there is more than one marriage, list the 1st mother above her children, leave a line after her children's names, then list the 2nd mother followed by her children.
Please do NOT send attachments without prior notice. Thank you!

* Put all SURNAMES in capital letters!
* DO NOT include LIVING PERSONS on these charts - if you do they will be removed.  Simply put the word Living in place of
    their name, including no other information on them.
* CUT AND PASTE the following chart into an E-MAIL and fill in the blanks!
* DELETE ANY EXTRA CHILDREN'S SPACES THAT YOU DON'T NEED!
* Add any additional information in the area left for notes, below the children.
